What is more likely to cause skin infections? Gram positive or gram negative?
Gram positive
Bacteria that may be commensals?
NOT PROBLEMATIC
- Staph epidermidis (coagulase negative)
- Corynebacterium
- Propionibacterium
If patient with no clinical question of infection comes back with a positive bacterial swab. Would you prescribe antibiotics?
ONLY swab if there is a clinical question of infection including in ulceration
Can have commensal bacteria
Some strains of staph aureus produce toxins, what one causes dermatological symptoms
SSSST - staph scalded skin syndrome toxin
Antibiotics that work on MRSA
Doxyxcycline
Clindamycin (one of 4C’s)
Co-trimoxazole
NOT flucloxacillin
Necrotising fasciitis
SEVERE PAIN
Deeper than cellulitis
Surgical debridement (cut open and antibiotics go right in)
