How can HPS be prevented?

A

Avoid rodents and take precautions cleaning areas where rodents are/have been

8
Q

Pathogenesis of hantavirus

A

Viral load in alveoli recruit macrophages and lymphoblasts –> secrete cytokines –> activate endothelium –> increased pulmonary capillary permeability –> severe pulmonary edema

9
Q

Viral load onset vs. progression of HPS

A

Very high viral load at onset of pulmonary edema that is then rapidly cleared but damage persists

16
Q

Why is hantavirus difficult to diagnose?

A

Early phase mimics influenza (“flu-like” sx)

17
Q

Key diagnostic characteristics of hantavirus

A

Rapid onset respiratory insufficiency + SOB + fever + fatigue + hx rodent exposure + tetrad of:

  1. Thrombocytopenia
  2. Leukocytosis with left shift
  3. Lymphoblasts on PBR
  4. Increased HCT
18
Q

Why is HCT elevated in pt with HPS?

A

Ultrafiltration of fluid into the lungs

19
Q

How to dx hantavirus

A

Serological assays of hantavirus-specific IgM, IgG or detection of hantavirus via RT-PCR

20
Q

Tx for hantavirus

A

Supportive

no tx, cure, or vaccine

21
Q

DO NOT DO WHAT IN HPS?***

A

Give excessive fluids trying to correct hypotension –> exacerbate pulmonary edema

22
Q

What supportive tx may be necessary for HPS?

A
  1. Beta1 agonist to correct BP/inefficiency of heart
  2. O2
  3. Ventilator/intubation
