What should you do before attending the Ward
Print off a stcok list for the ward on WellSKY
What should you do first when getting on the ward?
Look for handover sheet
If a patient is new what is the process to take
- include meds history process
Check chart to see if Meds history is done
Check clarking information to check reason for admission and key history
Check front of chart - allergies, Patient identifiers, Meds history/ reconciliation.
Check items on chart - see whats started and continued.
Speak to patient:
Introduce yourself
Check identifiers
Check allergies
Check medications that are continued on chart - Name & dose
Check OTC medications
Check herbals, vitamins, creams, inhalers
Check any medications form other clinics that they might attend.
If smokers - offer NRT
ask for permission to check GP records
Close off conversation
If NRT wanted write on handover and inform member of staff
Check GP record
What is the process to do when checking a GP record following a medication history
Check medications on their repeat match up with medications on chart
- if different dose write next to item on chart what the normal dose is
Check acute items, if patient no longer taking, write in meds history box on front: NAME, STRENGTH, QUANTITY, DATE OF SUPPLY, ACUTE
Check items prescribed within the last 3 months. Any prescribed that arent on chart write on front of chart.
If a CD is on the chart what should you write?
In the supply box put CD in a triangle
If a chart is rewritten what is the process to take
Med shistory shouldnt be needed for rewrite.
Check locker to make sure that patient has a supply of everything.- items that are stock write stock, items not stock put the quantity that the patient has.
If liquid or inhaler and looks like enough supply put QS (quantity sufficient)
If stock is not supply on ward what should you do
If patient doesnt have adequate supply (usually 14 days), order it on PTOS.
