AGRANULOCYTOSIS
Mech of action statin + during what time of day do you take statin
inhibits HMG-CoA reductase rate limiting enzyme inhepatic cholesterol synthesis (dec intrinsic cholesterol synthesis).
Night as cholesterol synthesis occurs during night time
monitoring statin
LFT every 3months and 24 months
contraindication of statin
Macrolides: erythromycin + clarithromycin esp stimvastatin
increases risk of rhabdomyolysis/ myopathy
CYP3A4 inhibited by clarithromycin therefore increasing level of statin
indication of statin, pri + sec intervention
indication:
ALL with CVD: IHD, stroke, TIA, PVD.
10 yr CV risk >10%
DM using QRISK 2, DM1: dx >10yr ago, >40yrs old, diabetic nephropathy
PRIMARY: atorvastatin 20mg
SECONDARY: 80mg OD following ACS
salicylate overdose Lab results + sx
Resp alkalosis -> later metabolic acidosis
Sx: TINNITUS, hyperventilation, sweathing/ pyrexia, N+V
salicylate mx
urinary alkalinisation w/ iv NaHCO3 inhances elimination of aspirin in urine
Haemodialysis indication:
serum conc >700mgL
Met acidosis resistant to tx
acute RF, pulmo oedema
seizure/coma
hydroxychloroquine Monitoring
used in RA + SLE
visual acuity monitoring annually
Bulls’ eye retinopathy: severe + permanent visual loss
