smell an essence with the right and left nostril individually, and try to identify what the scent is
t: optic 2
- vision acuity - cover one eye and read a snellen eye chart from 10 feet away (do to each eye) until you make a mistake
- peripheral vision - sit 2 feet away from your partner and cover left eye. hold a pencil 6 inch away and move it to the right. don’t track it and look straight ahead. say stop when it disappears from your peripheral vision. record the distance from left temple to pencil
t: oculomotor 3, trochlear 4, abducens 6
- eye movement: face your partner 2 feet away. hold ur finger 18 inches from their face, and draw a plus, and an x. have their eyes follow ur finger. eyes are supposed to move smooth together, and quick. not delayed or bouncing.
- pupil concentration: partner covers left eye and shiine pen light from 6 inches away. the pupil should immediately become smaller.
- pc pt 2: uncover both eyes and shine light into right eye then look at left pupil. both pupils should become small at same time.
t: trigeminal 5
- motor function: face partner and hold fingers on their temples. tell them to clench and release jaw 3 tims and feel for equal temporalis muscle contraction. then, hold ur fingers at the back of the partner’s jaw. ask them to clench and release jaw 3 times to feel for masseter muscle contraction. it should feel even on both sides.
- sensory function: face partner and they close eyes. lightly touch their forehead, nose, right and left temple, right and left cheek, and right and left jaw with a cotton ball. do the same thing with a cold/wet cotton ball, and ask if it is cold or not.
t: facial nerve 7
- sensory: place a q tip in a sugary solution. touch it on ur partners fron, middle, left, and right of their tongue. ask if they tasted sugar for each touch
- motor: ask partner to make these facial expressions, and check if it is symmetric on each side: smile, cross eyes, frown, close both eyes tight, raise eyebrows
t: vestibulocochlear 8
- cochlear - cover left or right ear. listen to a tuning fork at 329.6 hertz, and tell your partner to stop when they cant hear the noise anymore. repeat with 440 hertz and 523.3 hertz. the time should be the same and no more than a 5 second hearing difference.
- vestibular - have partner spin around 10 times with eyes closed, then immediately open them. their eyes should be moving for a few seconds.
t: glossopharyngeal 9, vagus 10
rest hand on partners throat and have them swallow. u should feel their throat move up and down bc the epiglottis opens and closes. have partner open mouth, and put a tongue depressor on it. use a q tip to lightly touch the uvula at the back of the throat, and they should gag/activate gag reflex. keep holding the tongue down, and have partner say “AH” then watch how the uvula moves. it should shake
t: accessory 11
- trapezius - put both hands on partners shoulders. have them shrug their shoulders as u put pressure on them. it should feel equal on both sides.
- sternocleidomastoid - put hand on right side of partner’s neck, and have them turn their head to the left. feel for contraction of sternocleidomastoid. repeat on other side. it should feel equal
t: hypoglossal 12
have partner stick out tongue. it should be in middle of mouth and evenly spread out. have them move it left right up and down. it should be quick and it should extend the same distance for each movement. then, have them say a tongue twister. write down any speech issues. the movement of the tongue is necessary to correctly form consonant and vowel sounds.
