What is Meckels diverticulum (RF, Sx)
Most common congenital malformation of bowel -remnant of failure to close a duct causing a pouch
most commonly asymptomatic, but 20% develop to diverticulitis
Rule of 2 -2% of pop, 2:1 M:F
2 feet proximal to ileocaecal in adults, 2 types of tissue (gastric and pancreatic), 2 inch long, 50% sx under 2
most common - M under 2 with rectal bleeding (acute periodic and painless)
rarely heamodynamically unstable
MASSIVE PAINLESS PR BLEED
can also present as obstruction -under 2 with constip, vom, nausea, irrubale, weight loss, poor feeding
palpable mass present
can even cause intussuception
finally can present as diverticulitis (20% of sx cases in ADULTS) - RLQ pain (like appendicitis), fever, anorexia, rectal bleeding,- care for perforation
ix and Mx of meckels diverticulum
Best test - Technetium 99 (meckels scan)
FBC -aneamic, leuko left shit (inflam)
AXR/CT -show the obstruction if there
USS- if intussuception
Mx -asympto -do nother
Sx -
bleed -surgical removal
Obstruct-surgery and remove adhersion
Perforate-excise and abx
