What is Intussuception (RF, Sx, Ix)
Prolapse of one part of intestine into lumen of distal part - most common illaocaecal
common cause of obstruction in children 3-12 months (peak at 9)
M>F, viral prodrome
Caused by “lead point” -a thing that causes intestine to attach and roll back
in children, these can be normal -eg peyers patches/hyperplasia of lymp nodes
in adults nearly always pathological lead point
Pt are 3-12, with
obstruction-colicky abdo pain, vomiting, abdo mass, abdo distention, poor feeding
Lethargy irritable, hypovalemic shock, poor feeding
Reducurrent jelly stool/Blood per rectum is the “key finding”
1st line when suspected - abdo USS- see TARGET sign
Xray also useful -but not perf
Diagnostic enema -meniscus sign
Mx-
fluid ressuc + contrast enema reduction (air enema or liquid puts in back in place)
2nd line -fluid and surgery
can give broad spectrum if needed -clindamycin/gentamycin
