Le Fort Fracture grade 1
from nasal septum –> lateral pyriform rims
horizontal
above teeth apices
crosses below zygomaticomaxillary junction
traverses pterygomaxillary junction to interrupt pterygoid plates
Le Fort Fracture grade 2
pyramidal shape
extend from nasal bridge/below nasofrontal suture through frontal process maxilla
Inferolat through lacrimal bones + inf orbital floor + rim through/near inf orbital foramen
Inf through ant wall maxillary sinus
Le Fort 3
start at nasofrontal + frontomaxillary sutures, extend post along med wall orbit through NL groove + ethmoid bones
continues along inf orbital fissure, continues superolaterally through lat orbital wall, through zyogmatic arch

CSF rhinnorrhoea
What is Superior orbital fissure syndrome
severe force to lat wall orbit –> compression NV structures
Sx Superior orbital fissure syndrome
opthalmoplegia + ptsosis
RAPD
Dilatation pupil + loss accommodation + corneal reflexes
Altered sensation from forehead to vertex
What is an orbital blow out fracture?
striken orbital rim
bone fragment = displaced downwards into antral cavity, remaining attached to orbital periosteum
Sx retrobulbar haemorrhage (PPPPV)
Pain
Proptosis
Pupil reactions = lost
Paralysis
Visual Acuity lost
Mx retrobulbar haemorrhage
mannitol
acetazolamide
dexamethasone
