a. Training Report (AF Form 475)
i. Used for training of 8 WEEKS OR MORE
ii. Filed upon completion, interruption, or elimination from training
b. Officer Performance Report (OPR)
i. Completed Annually
ii. Change of Reporting Official (CRO)
iii. MINIMUM 120 DAYS OF SUPERVISION
c. Letter of Evaluation (AF Form 77)
i. Used when ratee is away from reporting official
ii. CRO with less than 120 DAYS OF SUPERVISION
iii. Under supervision of someone other than your supervisor
iv. Provides input to rater
- STATE when OPRs are prepared.
a. COMPLETED Annually
b. CHANGE of Reporting Official (CRO)
c. MINIMUM 120 days of supervision UNDER SAME SUPERVISOR
- Identify inappropriate comments for performance reports.
a. Charges/investigations not completed or acquittals
b. Recommendations for decorations
c. Race, gender, age, religion, etc. of ratee
d. Drug/Alcohol abuse rehabilitation programs
e. Previous ratings or reports
f. Developmental education (completion or enrollment)
g. Advanced academic education
h. Promotion recommendations
i. Family activities/marital statuses
- Describe “best’ qualified and “fully” qualified promotions for officer personnel.
a. Fully Qualified:
i. For promotion from 2d Lt to 1st Lt
b. Best Qualified:
i. For promotion to Capt thru Col
ii. Promotion Zones
iii. “BEST QUALIFIED” BASED ON PERFORMANCE AND PERFORMANCE BASED POTENTIAL
