A
Partial sprain to Lisfranc ligament to complete tear and joint separation

Anatomy
A
- Lisfranc joint is where the mets join the rest of the foot at the cuneiform and cuboid bones
- Lisfranc ligament is plantar ligaments commonly injured, runs from medial cuneiform to 2nd metatarsal

Causes
A
- indirect trauma longitudinal force with plantarflexion
- Direct trauma, crush injury

Diagnosis
A
- Palpable tenderness to dorsal midfoot region
- pain is present in area for greater then 5 days
- swelling/bruising around midfoot
- aggravated forefoot weight bearing
- Piano keys test
- Radiology findings, weight bearing x-ray

DDX
A
- Ankle sprain
- Tarsal fracture
- Metatarsal base fracture

Prognosis
A
- Heals slowly
- Can be career ending

Treatment
A
- Needs to be treated ASAP for better outcomes
- Walking moon boot 6 weeks
- Orthotics and footwear to prevent midfoot collapse and flexion
- Reduce tightness via stretching/foam rolling
