Lecture 6 General Survey Flashcards
General Survey
Four Areas of whole person
- Physical appearance
- Body Structure
- Mobility
- Behaviour
Physical Appearance
ASLSF
Age Sex Level of Consciousness Skin colour Facial features
Body Structure
SNSPPB
Stature(height) Nutrition (weight) Symmetry Posture Position Body Build
Mobility
GR
Gait
Range of motion
Behaviours
FMSDP
Facial expression Mood and affect Speech Dress Personal Hygiene
BMI formula
Underweight, normal weight, overweight, obesity
Weight(kg) / height (m)^2
Underweight = <18.5
Normal weight = 18.5-24.9
Overweight = 25-29.9
Obesity = >30
Waist to hip ratio
Reflects body weight distribution
Waist circumference/ hip circumference
Abnormal
>= 1.0 in men
>= 0.8 in women
Waist circumference alone can be used to predict health risk
Vital signs normal ranges
Temp, pulse, RR, BP, SpO2
36-38 60-100 beats/min 10-20 breaths/min 120-139/80-89mmHg 95-100%
Temperature
Circadian rhythm = low in morning, high in afternoon
period/menopause = high in period, hot flashes in menopause
Up with exercise
Low in elders,
Up with stress
- 5 lower in axilla
- 5 higher in rectal
Wait 20 mins after pt drank
5mins after gum
2 mins after smoke
pyrexia = fever
Pulse
Palpable bounding blood flow
Irregular do full minute
Rate, Rhythm, force 0-3, equality
Respirations
Don’t tell pt your are observing, rise and fall = one resp
4:1 pulse rate to resp
resp rate, pattern, depth
Oxygen Saturation SpO2
% of hemoglobin bound with O2
Influenced by nail polish
Blood Pressure
Systolic over diastolic
Systolic = ventricular contraction Diastolic = Resting filling heart
Rest 5 mins
2,5 cm above artery
Cuff 40% arm circumference
80% arm length
Blood Pressure COmponents
Korotkoffs sounds
Phase 1 systolic pressure = clear tapping
Phase 4 = muffling
phase 5 = diastolic pressure (silence)
