A
- Variola virions are inhaled
- Virions travel/ are transported to lymphoid organs
- Virions infect macrophage and undergo early replication
- Infect macrophage enter the bloodstream

Smallpox Pathway: The skin
A
- Infected macrophage travel to the small vessels in respiratory mucosa and dermis
- Macrophage migrate to the epidermis and infect basal cells
- Basal cell infection leads to cellular necrosis and localize edema

Incubation Period
A
- Incubation period: 12 days (7-19 days)
- Virus undergoing early replication in macrophage within Lymphoid Organs
- NOT infectious

Prodrome Period
A
- Infected macrophage entering tissues
- Abrupt onset of fever >101 F
- Malaise, headache, muscle pain, nausea, vomiting, backache
- Becomes infectious during this period

Enanthem Period
A
- Mucous membrane lesions
- Appears – 24 BEFORE skin rash
- Becomes infectious from lesions in mouth

Exanthem Period
A
- Skin rash
- Macules, papules, vesicles, pustules, scabs
- Centrifugal distribution
