What is typical presentation of child with hip pathology?
complains of knee pain
What is prognosis of neurogenic scoliosis?
difficult to treat; not braceable
associated w/ multiple medical conditions
What is congenital scoliosis?
failure of segmentation or failure of correct formation
have variable prognosis; may need early surgery
What is infantile idiopathic scoliosis?
seen < 3 yo M > F
usually self limited
Rib vertebral angle is good prognostic factor
What is juvenile idiopathic scoliosis?
seen in 4-10 yo
curve progresses despite treatment –> need to fuse thoracic curves
What is adolescent idiopathic scolioisis?
seen in kids > 10 yo
do not have to worry about curve until 20-30 degrees; large curve more common in F
When do you fuse with scoliosis?
curve > 40 degrees
WHat is DDH?
developmental dysplasia of the hip –> see subluxation, dislocation
What are risk factors for DDH?
- breech position
- female gender
- first child
- family history
What are abnormalities associated with DDH?
- packaging problem –> stiff neck; feet turn in [metatarsus adductus]
- oligohydramnios
- tight uterus
How do you diagnose DDH?
ortolani + barlow
assymetric skin folds
toe walking
What is clubfoot?
congenital talipes equinovarus
bilateral in 1/2
What is etiology of clubfoot?
either idiopathic or secondary to spina bifida, congenital bands, arthrogyrposis = tight ligaments in entire body
What is treatment for clubfoot?
serial casting = best treatment
What are unique feat of kids bone?
- physis
- thick periosteum
- porous cortex
