Laminectomy
Removal of the vertebral spinous process to help relieve nerve root compression and give nerve more room
s/sx Root compression
The 3 Ps
Pain
Paresthesia (numb/tingle)
Paresis (muscle weaknes)
Laminectomy = Location
3 locations
-cervical (neck)
-thoracic(upper back)
-lumbar (lower back)
Cervical
Innervates diaphragm & arms
- assess breathing
-assess arm function
-watch for pneumonia
Thoracic
Innervates the gut/abdominal muscles
-Assess cough
-Assess bowels
-watch for pneumonia & paralytic ileus (paralyzed GI)
Lumbar
innervates bladder & legs
-assess urinary retention/last void/ bladder empty
-assess leg function
-watch for urinary retention
Post Op Laminectomy
1 answer is LOG ROLL
Mobilizing
-NO DANGLING
-no sitting for more than 30 mins
-supine to walking ASAP
-SITTING = BAD
-walking, standing, lying down =GOOD
Anterior thoracic laminectomy
only one that requires chest tube
Laminectomy w/ fusion
taking bone graft from iliac crest (hip) and spine.put in place so things are not bone to bone
2 Incisions
Hip -most pain, most drainage
Spine- risk of rejection
Both have equal risk of infection
Temp restrictions
4 restrictions for 6 wks
-no sitting for 30 mins
-lie flat log roll
-no driving
-not lifting above 5 lg
