Arterial Blood Gas Assessment
1) identify pH
2) Detn direction of PaCO2
3) Detn direction of bicarbonate
4) identify primary disorder - match pH shift with PaCO2 or bicarbonate
5) identify if there’s compensation/opposing pH
PaCO2
acid - respiratory, increases –> acidic, decreases –> basic
Bicarbonate
base - metabolic, increases –> basic, decreases –> acidic
Pulmonary Function Tests
via simple spirometry
FEV
forced expiratory volume
FEV1
forced expiratory volume in first second
FVC
forced vital capacity
FEV1/FVC
% of volume exhaled in first second, normal >/= 80%
Obstructive Disease
ex: Asthma, COPD LABS: FEV1 --> decreased FVC --> Normal/decreased *FEV1/FVC --> Decreased
Restrictive Disease
ex: Emphysema, expansion of lung prevented but lung is normal LABS: FEV1 --> decreased FVC --> decreased *FEV1/FVC --> Normal (>/= 80%) *PEF --> Normal
bronchodilator medications judged effective IF–>
> /= 12%improvement, and 200 ml improvement in FEV1
hypocapnia
decreased CO2 in blood
hypercapnia
increased CO2 in blood
