Bile
emulsifies fats, increases peristalsis, and retards putrification
Total bilirubin
composition –> conjugated, unconjugated, and delta bilirubin
Direct bilirubin
COmposition -> conjugated and delta bilirubin; water soluble forms of BR
Indirect bilirubin
Composition - unconjugated bilirubin(un is in)
insoluble forms of BR
Delta bilirubin
BR irreversibly bound to albumin; nontoxic; not excreted in urine or bile; long half-life
Pancreatic disease
Increased –> amylase and lipase
Cholestatic disease
Inc total bilirubin - initially
THEN –> INCREASED conjugated/direct BR, delta BR, GGT, alkaline phosphate
Cause- obstruction in bile pathway - bile reabsorbed, “bile stasis”
Hepatic damage from immoderate alcohol intake
induces hepatic enzymes and inc GGT, absence of liver disease
Hepatocellulary injury
damage but liver still fning;
LABS:
INC - AST, ALT, ALP, GGT, LDH, CK
NORMAL - albumin, PT, Ammonia, Bilirubin - total, unconjugated and conjugated
Impaired Hepatic Synthetic Function
liver no longer fning properly
LABS:
Normal/Inc – AST, ALT, ALP, GGT, LDH, CK,
DECREASED - Albumin*
INC - PT, Ammonia, Bilirubin(total and unconjugated)
