Sterile body sites
Blood, CSF, pleural fluid, urine, synovial fluid, pericardial fluid, peritoneal fluid
Individual isolates susceptibility report
Obtain specimen from patient of interest and have lab test susceptibility of organism to various abx. Move from broad spectrum treatment to targeted abx treatment.
Hospital susceptibility report
hospitals report indicating organisms isolated on inpt and susceptibility to abx. Allows emperic therapy
Normally colonizing bacteria
Skin, GU, GI, Resp
Blood culture
Seeking out systemic infection- blood = sterile. Interpret 24-48hrs
Sputum culture
Inducing sputum is hard. Testing for pneumonia or Tb. Determine specimen to cover organisms
Sputum Gram stain
Neither sensitive nor specific
Viral culture
Requires cell cultures- cytopathic changes indicate viral infection
Fungal culture
Sabiuraud’s agar-inhibiting bacterial growth. Takes weeks for fungal growth
Fungal Serology
Presence of Abs in serum or CSF- systemic mycoses. Rise must be significant
Acid Fast Stain
Mycobacteria. Neither gram + or -. M. Tb, M leprae, M avium
Mycoplasma serology
Clinical Dx of M. Pnemoniae
Cold agglutinin test
M. pneumonia, adnovirus, mono- cause autoimmune Abs that agglutinate at 4 C
Determine bacteremia
Blood culture- results 24-48 hrs
