KACH: Antiemetic Drugs Flashcards
When neausa and vomiting are due to stimulation of vestibular receptors use these medications
Antihistamines & Anticholinergic
Antiemetics work
Locally
Centrally
Locally: decrease local response being sent to the medulla to induce vomiting
Centrally: blocks the chemoreceptor trigger zone CTZ or suppresses the vomiting center directly
Chlorpromazine (generic)
Perphenazine
Prochlorperazine
Promethazine
Class of medication
Use
Phenothiazines
Treats neausa/ V Post Op
Have a psychotic effect too
Promethazine has this indication
N/V
Motion sickness
Phenothiazines are central acting antiemetics that change responsiveness or stimulation of the ______ in the medulla
CTZ
Indications
N/V associated with Anesthesia
Intractable Hiccups
Phenothiazines
Contradictions
Don’t use in patients with brain damage due to further risk of CNS depression
Phenothiazines (Promethazine) -Zine
Adverse Effects
HypoTNS
Cardiac arrhythmias
Dry mouth
Tardive dyskinesia
Neuroleptic malignant syndrome
Photosensitivty
Urine: pink - red-brown
Phenothiazines (Promethazine) -zine
Treats
N/V associated with emetogenic cancer chemotherapy, radiation
Post op N/V
Action: blocks receptor sites associated with N/V peripheral & in the CTZ
5- HT³
Granisetron
Ondansetron
Palonosetron
This type of drug
Indications
5 HT³ Receptor Blocker
Treatment of N/V associated with chemotherapy
These drugs act directly on CNS to block N/V but have little to no effect in the serotonin, dopamine, or corticosteroids receptors
Substance P/Neurokinin 1 receptor Antagonist
Aprepitant
Fosaprepitant
Dimeglumine
Rolapitant
Work on CNS but have little affect on serotonin, dopamine, corticosteroids
Substance P/Neurokinin 1 receptor Antagonist
These type of Antiemetics have the adverse effects of
Neutropenia
Anemia
Leukopenia
Substance P / Neurokinin 1 receptor Antagonist
Aprepitant & fosaprepitant (Substance P / Neurokinin receptor Antagonist)
Have this adverse effects
Decrease effectiveness of hormonal birth control
Substance P/Neurokinin receptor Antagonist
Have restrictions on combination
Rolapitant cannot be combined with thioridazine or pimozide
Aprepitant & fosaprepitant should not be used with pimozide
