A prescription COPY must include:
- Pt’s name and address
-prescriber/practitioner’s name
- Drug name, strenght, quantity and sig
- date of 1st and last DISPENSE
- name and address of community pharmacy
- # Authorized Refills REMAINING***
- the signature of the REGISTRANT PROVIDING THE COPY.
- a clear indication that the document is a COPY.

• What are the requirements for transfering an Rx? What must be docuemnted on the pt record?

Answer 2

Transferring Rx Requirements:
- Rx info (drug, sig etc)
- the following must be documented on the pt record:
i) date of transfer
ii) registrant ID (sender)
iii) ID of the CMTY pharmacy TO which the Rx was transferred TO.
iv) ID of person WHO RECIEVED the transfer.
