What are the 8 components to be considered a COMPLETE RX?

1) Date
2) Name of pt and 1 other identifier.
3) Prescriber Name, address, and phone #
4) DRUG name, dose form, strenght.
5) sig: roa and instructions
6) Mitte : total QUANTITY to be dispensed
7) # of Refills -> and interval if applicable.
8) Signature of prescriber (if written); (if VERBAL) License number and intitial of registrant who took the Verbal order.
Can verbals be taken from phone messages?
yes!
When can an Rph adjust the quantity of drug to be dispensed?
4 reasons - based on pt request or package size.
what are some reasons where an rph may use their judgement to only give a pt a smalelr amount.
i) if pt requests a SMALLER AMOUNT.
ii) if manufacturers unit of use standard package size doesnt match prescribed quantity (can give a bit more)
iii) if quantity prescribed exceeds amount covered by a drug plan (usually only cover up to 3 mos)
iv) or if pt only wants a TRIAL.
v) Rph may adjust quanitty if :
- they consult with prescriber AND if ther’s a hx of poor copmliance, drug misuse, or pt is at risk for overdose.
