What are the 7 requirements all Rx Drug LABELS must include?

If a label is too small to include all 7 items, what are the minimum 4 requirements?

A

1) Full name of PATIENT

2) Name, address + Phone # of Pharmacy.

3) Rx # + Dispensing date

4) Name of prescriber

5) Name of drug, quantity and strength of drug.

6) Directions for use

7) Any other info required for good Rx practice.

*If container too small for complete label, it must include:
- Rx # and dispensing date
- full pt name
- just NAME of drug and
- a complete label must be attached to a larger container and the smaller container placed inside the bigger one.
