When giving fluids for sepsis (in absence of PET/renal disease), what treatments should be given?
Aim SBP >90 or MAP >65mmHg
UO >0/5mls/kg/hr
CVP 8-12mmHg
1st line Abx for women who is septic not critically unwell and unknown source?
Co-amox + Met
or
Cef + met
Sepsis Tx if Group A Strep suspected?
Clindamycin
Sepsis and risk for MRSA
Teicoplanin or linezoolid
Abx for postpartum endometritis
Clindmycin + Gent
Sepsis bundle
Blood culture
Urine output
Fluids
Antibiotics within 1 hour
Lactate
Oxygen >94%
When to consider vasopressors
If fluids do not maintain MAP >65 or if lactate >4
Commenst cause of postpartum sepsis
Endometritis
If septic which analgesia should you avoid?
NSAIDs - effect renal function, coagulation
Abx for severe sepsis
Tazocin + Genty
or
meropenem + gent
+/e metronidazole
