physical: fever, occult stool +
historical: weight loss, older, nocturnal awakening, IBD history
labs: decreased Hgb, increased WBC, increased ESR

antispasmodics
anticholinergics
Ca++ antagonist
Drugs to treat diarrhea
Loperamide
Diphenoxylate
Cholestyramine
Drugs to treat constipation
Fiber Osmotic laxatives Cisparide Misoprostil PEG solution
Two serotonin drugs for IBS
for IBS-Diarrhea, ALOSETRON (serotonin 3 receptor agonist). reduces perception of pain, induce rectal relaxation, delay colonic transit.
for IBS-Constipation, MISOPROSTIL (serotonin 4 receptor ANTAGONIST), prokinetic, stimulates peristalsis
Psychological therapies for IBS
antidepressants: tricyclics for IBS-D, SSRI for IBS-C
