What are the 4 groups of people who should be on statins
- Anyone with ASCVD
- Adults with LDL > 190mg
- 40-75 y/o with DM and an LDL with 70-189 mg/dL
- 40-75 y/o with 10 year risk greater or equal to 7.5%
What are the treatment recommendations and for anyone with ASCVD and adults with an LDL > 190 mg/dL
High intensity statin- 50% reduction in LDL
- Atorvastatin 40-80mg
- Rosuvastatin 20-40mg
What are the treatment recommendations for patients with DM and LDL of 70-189 mg/dL
Moderate intensity statin=30-50% reduction of LDL
- Atorvastatin 10-20mg
- Simvastatin 20-40mg
- Pravastatin 40mg
What are the treatment recommendations for 4-75 y/o with a 10 year risk factor of 7.5% or more
Moderate to High intensity statin
What are criteria for Therapeutic Lifestyle Changes?
- Approach - start small and encourage pt to make a goal
- Start a conversation and follow up
- Encourage change for health, lipids, BP,
- Diets and visual handouts
What is Familial Hypercholesterolemia?
LDL receptors are absent or dysfunctional
- Homozygotes = LDL 8x normal
- Heterozygotes = LDL 2x normal
What is Familial Hyperchylomicronemia
Lipoprotein lipase abnormality-the enzyme that helps peripheral tissue to take up TG from chymomicros and VLDL is disrupted
