A
- have you been admitted to hospital overnight before?

- immunisations?
- birth history?

A
- are you allergic to anything?
- allergic to any medicines?
- allergic to any foods?
- have you had an allergic reactions to anything else? (pets, pollen, other animals)

what would be asked in birth history?
A
- account of pregnancy and birth
- where were they born?
- were they brown on time or were they premature?
- what did they weigh at birth?
- any antenatal problems? (exposures, drugs, etc)
- was it a normal delivery/birth? (forceps, ventouse, is a C section, was it planned or elective?)
- were they admitted to SCBU/NICU? If so, why? how long for? what treatment fit they receive? (ventilation, CPAP, antibiotics, oxygen phototherapy)
- any other postnatal issues for mum or baby?

what should be asked about for feeding?
A
- were they breast fed or bottle fed? (if breast fed, for how long?)
- bottle fed (what product, what volume, what frequency?)
- for older children, ask them to run through a typical day to get a better picture

what should be asked in developmental history?
A
- aim to establish the time of achievement of milestones of motor, vision fine motor, social/adaptive, and hearing/language skills
- document any delays or help received such as speech and language therapy
- for older children, find out about school attainment

what age is a normal time to learn to walk?
A
roughly 1 year
-normal to be a bottom shuffler for up to 18 months
