What is an acetabulum labrum?

A

Ring of cartilage that surrounds the acebulum
Anterior portion most vulnerable to tears
Allows head of femur to articulate with the pelvis

What is osteoarthritis of hip

A

Deteriorating cartilage= rubbing bone= possible bone spurs

Risk= age, family history, injury, history

Osteoarthritis Sx

A

Anterior groin pain, pain worse with time, worse with activity, stiffness= difficulty walking/bending, locking joint, crepitus, worse with rainy weather

How can you distinguish hip pain with SI/lumbar issues?

A

Hip is anterior
SI/lumbar is posterior
(Patients often point posterior and say they complain of hip pain)

What is Ankylosing Spondylitis

A
  • Systemic rheumatic dz, chronic inflammatory dz of axial skeleton
  • spine starts fusing at SI joint-> cranial
  • Unknown cause
  • genetic

Sx of Ankylosing spondylitis

A
  • gradual onset, peak around 20-30 yrs
  • initial chronic dull achy back/glute stiffness
  • AM stiffness, worse with rest, better with movement
  • Systemic Sx= weight loss, fever, fatigue
  • eye pain
  • inflammation of the prostate
  • cardiovascular/LU involvement

SX of sacroiliac dysfunction

A
  • dull unilateral low back pain, isolated around PSIS
  • pain stiffness worse prolonged sitting/walking
  • inability to roll off back once supine
  • severe= referred pain into hip/groin/low leg (rarely below knee)

  • very uncommon
  • maternity
  • joint not fully developed until mid twenties, most occurance a happen prior to this age

Hypo mobility SI (4) kinds/causes

A

1) up slip= fall on buttocks
2) down slip= forced pull on leg (less common)
3) anterior rotation= tight hip flexor a, increased lumbar lordosis, high heels
4) posterior rotation= tight hamstrings, decreased lumbar lordosis

A

Sx= asymmetric gluteal folds, limb length inequality, pigeon toed/w-sit

A

1) congenital- hormonal (relaxin), females, genetic (indigenous)
2) aquired- swaddling infants with legs together, breech birth

What is an acetabulum Labrador tear

A
  • cause= high impact, SHI ROM at hip
  • Sx= asymptomatic, pain in groin, clicking in socket with movement, stiffness, decrease ROM hip
  • risks= reg. Hip rotation (golf, ballet), high impact, osteoarthritis
  • *75% insidious
