What is a hernia?
Projection of viscera through a normal or abnormal opening in the abdominal wall
What is the most common inguinal hernia?
Indirect Inguinal hernias (2/3 of hernias)
Direct hernias major facts
Usually in older men, protrudes through Hesselbachs triangle, goes directly through superficial ring next to spermatic cord, medial to epigastric vessels, weakness in abdominal wall
Indirect hernia major facts
MORE COMMON than direct! Go through BOTH deep and superficial, comes out WITHIN spermatic cord and not beside it, happens more often in younger fetus or younger men
What is unique to indirect hernia?
They go through both the deep and superficial ring of inguinal canal and end up within the spermatic cord
How do you test for hernias in male physicals?
A fingertip is injected into superficial ring to feel for bulge, is there is no hernia this doesn’t hurt but it WILL hurt if there is a hernia, this test does NOT indicate if the hernia is direct or indirect
What type of hernia forms in Hesselbach’s triangle?
Direct inguinal hernias
What forms the Hesselbach’s triangle?
Inferior epigastric vessels, lateral border of rectus abdominus muscles, and iliopubic tract ligament
