mechanism of action
A
- Antithrombin III inhibits coagulation by neutralizing the active clotting factors (serine proteases)
- Heparin increases the affinity of antithrombin III for clotting factors to greatly increase their rate of clotting factor inactivation

Caution in hepatic/renal impairment

- Haemorrhage - discontinue drug, administer protamine sulfate IV if necessary
- Heparin-induced thrombocytopenia
- Hyperkalaemia
