The purpose of the Health Center Tier System Protocol policy is
to reduce the risk and burden of fire service occupational morbidity and mortality while
improving the welfare of firefighters.
The Health Center Tier protocols are based on NFPA ____ and NFPA ______

The medical components of the annual physical include: medical history, physical
examination, blood testing, urinalysis, hearing test, pulmonary function test, chest x-ray (every___ years
or as medically indicated), electrocardiogram (30 years or younger every ____ year, 31-39 every ____
year, 40 and over _____), immunization record review and TB testing.

Tier 1 is
the minimal health parameters to which Glendale Fire Department members should
maintain for field conditioning.
Tier 2 is i
indicative that health issues are noted and interventional support or change is
recommended. It is recommended that you reverse trends and stay healthy.
Tier 3
is indicative of health issues sufficient for mandatory referral for wellness/fitness
intervention, but removal from the field is not yet required. You need to participate in a health
regiment.
Tier 4
is indicative of health issues sufficient enough to mandate removal from the field, at the
clinician’s discretion, and mandatory referral for wellness/fitness intervention.
When a Glendale Fire Department member, regardless of rank, is between _____ METS for the
first time, they remain on shift (as long as they do not demonstrate any clinical indications of
cardiac disease or other medical risk factors) and are offered resources to assist with physical fitness
and nutrition (if needed).

The member
will be given ____months to get their METS to ____or greater.

If they show no improvement, they are
place on a 40 hour assignment if after another ___ months, they still show no signs of improvement; a
progressive discipline process will start. The member does not have to take the 40 hour option, but
can use their vacation and sick leave instead.

Any Glendale Fire Department member falling below ___METS is immediately taken off the truck and
placed on 40 hour assignment.
