Features of trigeminal neuralgia
Recurrent short episodes of severe stabbing pain affecting one side of the face in the trigeminal nerve distribution
Typical age presentation of trigeminal neuralgia
> 50 years old
Triggers of pain in trigeminal neuralgia
Touching face, eating or talking
Diagnosis of trigeminal neuralgia
MRI needed to exclude tumours and aneurysms
Treatment of trigeminal neuralgia
Carbamazepine first line, microvascular decompression surgery
Symptoms of headache from raised ICP
Worse in morning, when bending over, improve after vomiting and lying down. May be associated with neurological deficits
Differentials of headache
Migraine, cluster headache, tension headache, trigeminal neuralgia, raised ICP, giant cell arteritis, medication-overuse, meningitis
Symptoms of cluster headaches
Recurrent attacks of sudden-onset unilateral periorbital pain
Associations with cluster headaches
Watery and bloodshot eye, lacrimation, miosis, rhinorrhoea, ptosis, lid swelling, facial flushing
Time course of cluster headache
15 minutes - 3 hours. Once or twice a day over 4-12 weeks. Pain free months before next cluster
Treatment of cluster headache
Avoid triggeres, prophylaxis with verapamil
Treatment of acute attack of cluster headache
100% oxygen via non-rebreather mask, with SC or nasal triptan
Symptoms of tension headache
Bilateral non-pulsatile headache, tight around head
Associations with tension headaches
Scalp muscle tenderness
Treatment of tension headaches
Paracetemol or NSAID and addressing cause of stress
