Inspection of hand
trauma, lesions, color, swelling, Herberden’s or Bouchard’s nodes, asymmetry in muscles
ROM for hand
wrist: flex, ext, ulnar and radial deviation
thumb: flex, ext, abd, add, opposition
fingers: DIP and PIP flex and ext
Strength for hand
grip strength
Palpation
pulses
skin temp, masses, thickening
joints: swelling, tenderness, laxity of collateral ligaments
tendons: crepitus
Special tests
Flexor/Extensor tendon exam Phalen's Tinel's Finkelstein maneuver Thumb CMC grind test Allen's tests 2 point discrimination test
Flexor/Extensor tendon exam
Individually evaluate each tendon for intergrity
Flexor digitalis superficialis and profundus, flexor pollicis longus
Phalen’s test
touch dorsum of hands together for 60 seconds, parasthesias should occur
+ carpal tunnel syndrome
Tinel’s test
tap on the median carpal ligament to send a tingling sensation into hand
+ carpal tunnel syndrome
Finkelstein’s Maneuver
thumb in palm and ulnar deviation
+ DeQuervain’s tenosynovitis
Thumb CMC grind test
axial pressure and move in circle
+ pain indicates CMC joint arthritis
Allen’s test
asymmetry indicating isolated radial or ulnar artery lesion
2 point discrimination
trauma assessment
