ACUTE tx of MIGRAINES (either alone or in combo with acetaminophen, asa, caffeine) and their CI’s.

A
  1. analgesics (inhibits COX (COX2 is preferred))
  2. barbs (used when traditional therapies dont work/CI): SE - drowsiness/dizziness
  3. opioids (infreq HA’s (for pts who wake up with HA)) (pregant women –> use codeine or meperidine with caution)
  4. corticosteroids
  5. Ergotamines (specific for migraines) (CI in women planning pregnancy, uncontrolled HTN, sepsis, renal/hepatic failure, vascular dz)
  6. DHE (dihydroergotamines): same CIs as 6, but less rebound HAs than 5, and is injectable
  7. Sumatriptan (premier option): CI in vascular dz, uncontrolled HTN, complicated migraines; injectable.
  8. onabotulinum toxin: for acute tx of chronic migraine HAs.
