What is the testing method for chlamydia?

A

NAAT swab testing (vulvovaginal) for vaginas and first bit of pee from men - held for at least 45 mins

Picks up dead chlamydia- so will give a false positive eg if have been treated

A

Uncomplicated = Doxycycline 100mg bd for 1 week. Don’t need to take coil out

Preg/allergic = azithromycin (1 dose stat then 3 days)

NO SEX until 1 week after

Through direct mucous membrane contact/fluid contact

Females may not have symptoms

Men = green/yellow thick mucous

How do we detect gonnhorea and treatment

A

NAAT swab - same as chlamydia

BUT confirmed or suspected cases needs culture to confirm sensitivities (bact

Do test if cure 2 weeks after.

Treatment
1g ceftriaxone IM stat (into glutes)
OR ciprofooxacin PO ONLY if sensiitive on culture

NB alternative available, but only for severe penicillin allergy

Abstain for 7 days after treatment finishing

Mycoplasma genitalium testing, who do we treat?

A

NAAT swab
Treat and test only those with symptoms as a fair amount of people have it without any pathogenic parts
SOO kinda second line test

Uncomplicated 7 days doxycycline, azithromycin

Complicated = moxifloxacin (strong and harmful)

Abstain for 2 weeks and until symptom free

A

Levels are wayyyy too high and it overwhealms the tests and produces a false negative

A

Benzyathine penicillin

If true allergy doxy

Neuro/opthalmic = needs to stay in hospital for 2 weeks and need steroid as overwhealming cytokines response when Syphillis dies and will lead to swelling
