Colchicine Allopurinol Febuxostat PEG-uricase Probenecid
Colchicine - indication
High dose - treatment of acute gouty arthritis
Low dose - prevention of recurrent gouty arthritis
Colchicine - MoA
Prevents microtubule polymerization
Prevents Neutrophil chemotaxis to gout site
Colchicine - adverse effects
- GI (nausea/vom/pain)
- Hematologic (agranulocytosis, aplastic anemia, thrombocytopenia)
- muscular weakness
*Dose related and exacerbated with hepatorenal disease
Allopurinol - MoA
inhibits xanthine oxidase to decrease uric acid production

GI
Acute gout attacks
Rash
Allopurinol - serious rns
Fever, rash, toxic epidermal necrolysis Hepatotoxicity, marrow suppression Vasculitis Drug interactions (amp, thiazides, mercaptopurine, azathioprine) Death
Allopurinol - extremely serious problem
hypersensitivity - skin rash appears first and may progress to toxic epidermal necrolysis and death
Renal impairment increases chances
Febuxostat - MoA
oral xanthine oxidase inhibitor
Febuxostat - adverse
minimal
can be used with renal impairment
PEG-uricase - indication
treatment resistant gout (intravenous)
PEG-uricase - MoA
speeds resolution of tophi
Probenecid - indication
Mild gout
Probenecid - MoA
blocks tubular reabsorption of uric acid
Probenecid - adverse effects
nephrolithiasis (dont use in renal disease)
