what do we use to treat the pain of gout
indomethicin, naproxen or sulindac. intraarticular steroids.
colchicine
binds to tubulin and decreases microtubular polymerization. decreases LTB4. decreases leukocyte and granulocyte migration.
SE of cholchicine short term
diarrhea, GI pain
long term SE of cholchicine
hematuria, alopecia, myelosuppression. gastritis, peripheral neuropathy
what is the goal of prophylaxis for gout
decrease the uric acid pool
allopurinol
inhibits xanthine oxidase as a suicide inhibitor and decreases the purine pool and thus uric acid
SE of allopurinol
rash and hypoxanthine stone.
what is the most important interaction for allopurinol
inhibits the metabolism of 6-mercaptopurine and causes this to build up in the serum. when used efficiently this can decrease the dose of chemo used.
probenacid
inhibits proximal tubule reabsorption of urate.
SE of probenacid
inhibits the secretion of other acids as well. when there is high excretion of uric acid there can be crystalization
