GI drugs Flashcards
H2 blockers
cimetidine, ranitidine, famotidine, nizatidine
cimetidine toxicity
potent CYP inhibitor - multiple drug interactions
antiandrogenic effects - prolactin release, gynecomastia, impotence, decreased libido
crosses BBB - confusion, dizziness, HA
crosses placenta
proton pump inhibitors
omeprazole, lansopraxole, esomeprazole, pantoprazole, dexlansoprazole
PPI toxicity
increased risk of C. difficile infection and pneumonia;
hip fractures
decreased serum Mg with long term use
bismuth moa
binds to ulcer base, providing physical protection and allowing HCO3- secretion to reestablish pH gradient in the mucous layer
misoprostol moa
PGE1 analog; increases production and secretion of gastric mucous barrier and decreases acid production
misoprostol toxicity
diarrhea; abortifacient
octreotide moa
long acting somatostatin analog
octreotide use
acute vatical bleeds, acromegaly, VIPoma, and carcinoid tumors
octreotide toxicity
nausea, cramps, steatorrhea
aluminum hydroxide toxicity
constipation and hypophosphatemia; proximal muscle weakness; osteodystrophy; seizures
calcium carbonate toxicity
hypercalcemia and rebound acid increase
magnesium hydroxide toxicity
diarrhea, hyporeflexia, hypotension, cardiac arrest
osmotic laxatives
Mg hydroxide, Mg citrate, polyethylene glycol, lactulose
Tx of hepatic encephalopathy
lactulose - metabolized by gut flora to lactic acid, which promotes nitrogen excretion as NH4
