GI Flashcards
What is the best initial diagnostic modality for H. pylori?
The best initial diagnostic modality that is the most sensitive for H. pylori is a serology study.
Which test is the most appropriate for determining the eradication of H. pylori?
The best test to determine the eradication of H. pylori is a
urea breath test.
What is the preferred test for celiac disease?
the preferred test for celiac disease are IgA and Tissue Transglutamase antibody.
Testing for celiac disease should be considered in patients with GI symptoms including chronic or recurent diarrhea, malabsorption, weight loss, and abdominal distension or bloating.
Patients with a positive serology and high probability of idsease should undergo upper endoscopy with small bowel biopsy to confirm the dx.
bloody diarrhea with WBC
what is the most sensitive diagnostic test?
Invasive diarrhea causes leukocytosis, fever, bloody stool, and abdominal pain
the most sensitive diagnostic test is a lactoferrin
What organisms can cause BLOODY DIARRHEA?
Bloody Diarrhea can be cause by
MESSY CACA
Medical disease
E. Coli
Shigella
Salmonella
Yersinia histolytica
C. diff
Amoeba histolytica
Campylobacter
Aeromonas
What is the treatment for H. pylori?
The treatment for H pylori should include
a PPI or an antidiarrheal plus 2 antibiotics
OAC= Omeprazole, amoxicillin, clarithromycin
LAC= lansoprazole, amoxicillin, clarithromycin
A baby presents with dark urine, pale stools, and an elevated direct bilirubin. What is the most likely diagnosis?
The most likely cause is biliary atresia.
What are complications of Crohn’s Disease?
Complications of Crohn’s disease include
Vitamin B12 deficiency
Lab results associated with a B12 deficiency include:
macrocytic inemia, decreased serum cobalamin. Serum methylmalonic acid (MMA) and homocystein can also be elevated.
B12 binds to intrinsic factor (produced by the parietal cells) and is absorbed by the terminal ileum. In patients with Crohn’s the terminal ileum is compromised causing malabsorption.
When do you send a stool sample for a bacterial culture?
Send a stool sample for a bacterial culture in cases of bloody diarrhea associated with a fever.
what’s the most common cancer or the oral cavity?
The most common cancer of the oral cavity is
squamous cell carcinoma
What are the extraintestinal manifestations of Crohn’s disease
Enteropathic arthropathy, a nonerosive arthritis of peripheral joints and/or the axial spine occur concomitantly with bowel disease.
The GI S/S of Crohn’s include:
non-bloody, non-purulent chronic diarrhea with diffuse abdominal pain relieved by defecation.
Confirm the diagnosis of Crohn’s disease with a colonoscopy with ileoscopy and biopsy
What tumor marker is elevated in patients with colorectal cancer?
Carinoembryonic antigen (CEA) is a tumor marker that is elevated in patients with colorectal cancer.
CEA may also be elevated in femal patients with different gynecologic cancers
