What is Gastritis?
Inflammation of gastric mucosa
Can be acute or chronic
What casues Gastritis?
Result of a breakdown in gastric mucosal barrier: Mainly caused by medications
What drugs are risk factors for Gastritis and why?
Direct irritating effect on gastric mucosa
NSAIDs, including aspirin and corticosteroids, inhibit prostaglandin synthesis: increases acid and eats at the mucosa
What are diet risk factors?
Alcohol, spicy food: 99% from spicy food
Other risk factors?
Microorganisms- Helicobacter pylori
Bacterial, viral, and fungal infections
Auto immune
Environmental factors: Smoking, radiation
Pathophysiologic conditions
Other factors
Psychologic stress, NG tube, sepsis, burn victims: on meds at acute to help prevent stress induced ulcers
Acute Gastritis
Clinical Manifestations
Self limiting
Anorexia
Nausea
Vomiting
Epigastric tenderness
Feeling of fullness
Hemorrhage
Common with alcohol abuse
May be only symptom

Diagnostic Studies
Diagnosis most often based on history of drug and alcohol abuse
Why is Endoscopic examination with biopsy important?
important for definitive diagnosis
How do you test for H pylori?
Samples of breath, urine, serum, stool, and gastric tissue to detect presence of H. pylori: will show positive urease
What studies are not helpful?
Radiologic Studies
What does the CBC look for?
H&H
Why do occult blood tests?
Lower GI bleeed
What do serum antibody tests, test for?
parietal cells and intrinsic factor
What vitamin icreases gastric acid
Vitamin B12
What is used in severe cases and why?
NG tube—in severe cases
Observe for bleeding
Lavage to flush precipitating agent from stomach
Keep stomach empty and free of noxious stimuli
Can lavage, see gastric content, and to keep I&O
