Class
Loop diuretic
Actions
Inhibits sodium and chloride reabsorption in the proximal tubule and loop of Henle, which draws water with it and causes diuresis. This diuresis effect occurs about 10 minutes after the IV dose.
Direct venodilating effects which decrease venous return to the heart (decreasing preload). This effect is seen before the diuresis effect.
Indications
Acute cardiogenic pulmonary edema.
Hypertensive crisis.
Chronic CHF.
Hypercalcemia.
Contraindications
Allergy to sulfanomides.
Dehydrated patients.
Anuric patients.
Electrolyte depletion.
Side effects
Hypovolemia/orthostatic hypotension
Transient deafness/vertigo/tinnitus
Muscle cramps / weakness
Electrolyte problems: sodium, potassium, calcium, and magnesium depletion
Signs of hypokalemia: postural hypotension, malaise, fatigue, leg cramps, weakness.
Precautions
Auscultate lungs before and after administration.
May potentiate the effects if digitalis.
