Sympathomimetic
Alpha and beta adrenergic agonist
Endogenous catecholamine
Actions
Causes positive inotropic, chronotropic, and dromotropic effects (beta effects)
Bronchodilation
Peripheral vasoconstriction (alpha effect) which increases the coronary perfusion pressure
Indications
ACLS refer to treatment algorithms (asystole, pea, v fib, symptomatic brady)
Bronchospasm (asthma)
Anaphylaxis to relieve bronchoconstriction and prevent degranulation of mast cells
Contraindications
Hypovolemia before fluid replacement
Hypersensitivity
Hypertension
Coronary insufficiency (may worsen ischemia)
May worsen tachycardias and dysrhythmias like v fib and v tach.
Side effects
HA N/V tremors anxiety HTN tachycardia nervousness arrhythmias dilated pupils Prolonged use may result in acidosis due to serum lactic acid increases from decreased circulation due to arterial constriction.
Precautions
Digoxin and sympathomimetics increase the risk of arrhythmias.
Inactivated in alkaline solutions.
May increase myocardial oxygen demand as a result of tachycardia.
Inform RN of IV infiltration because it can cause tissue sloughing and phentolamine is needed to counteract.
Pts on beta blockers may be resistant to EPI or may have an increase in unopposed alpha effects
