What disease is caused by Naegleria fowleri?
Primary amebic meningoencephalitis
Intense HA, sore throat, fever, stiff neck, Kernig sign
Hemorrhage, edema and tissue necrosis
Rapidly fatal, 4-5 days
How does N. fowleri enter the CNS?
Nasal inoculation, penetrates cribriform plate and enters the CNS
How is N. fowleri diagnosed?
Purulent CSF with erythrocytes and amebae
CSF wet mount
Hemorrhagic CSF
What disease is caused by Balamuthia mandrillaris?
Granulomatous amebic encephilitis
Causes single/multiple brain abscesses
How is N. fowleri treated?
IV +/- intrathecal amphoteracin B
How is B. mandrillaris acquired?
Inhalation of cysts or direct skin contact
Typically in immunocomrpromised
What are the symptoms of B. mandrillaris infection?
Derm/nasal lesion
Edema of brain tissue
Fever, headache, nausea, vomiting seizure
How is B. mandrillaris diagnosed?
Autopsy
CSF
Serology/PCR?
What does Acanthamoeba cause?
Granulomatous Amebic Encephalitis
Acanthamoeba keratitis
What is Acanthameoba keratitis?
Corneal trauma with exposure to acanthamoeba
Causes corneal ulceration and severe ocular pain
Sight threatening
How is acanthamoeba diagnosed?
Brain biopsy
Neuroimaging
Corneal scrapings
How is acanthamoeba treated?
GAE - Combination antibiotics, antifungal, antiprotozoal
AK - topical antiseptics, repeated corneal transplant, enucleation

Sinusitis
Developed chronic meningoencephalitis
