Indications for fitness for duty exam (4)
- return to work (from workers comp or FMLA)
- job performance (employer concern or employee requesting accommodation)
- post employment offer (after hire as to not discriminate but can then dismiss)
- Medical surveillance exam (periodic for employees exposed to hazardous materials)

FMLA (family and medical leave)
Requires covered employers to provide employees with job protection for unpaid period for reasons such as pregnancy, foster care, family illness, military, etc. for up to 12 weeks in a 12 month period

Funcitonal capacity eval (FCE)
Set of tests, practices or observations combined to determine the ability in a variety of circumstances in an objective manner, performed if provider is unsure of patient capabilities or having difficulty with limitations, performed by PT or OT

- employer can have info if pertinent to job responsibilities or if healthcare release in nonworkers comp case
- all worker’s comp cases
Which party does the PA represent in fitness for duty exams?
-Both employer and employee
4 categories of return for duty
1) return to full duty (without restriction)
2) return to duty with permanent or temporary restrictions
3) Temporarily unfit for duty needing re-eval
4) Permanently unfit for duty
Return to work restrictions
-Write limitations based on actual medical condition, be specific about restrictions and do not just put people off work, discuss restrictions with the patient and employer (may have other options)
Scheduling a follow up for a fitness for duty physical patient
- schedule sooner rather than later
- refer to a specialist sooner than later
