Most common problem among food service and construction workers
-drug abuse
Age group worst affected by drug addiction
18-29
Regulated vs unregulated drug teseting
Regulated is the standard under dep. of transportation models and tests for marijuana, cocaine, opiates, amphetamines, and PCP, some have instant screen only that if neg can tell employee but if positive must be confirmed by sending to health and human services (HHS) lab and do not inform employee, medical review officer must monitor collection of specimen in regulated system, if unregulated must have clear policy and employees must be made aware of policy
Reasons for getting drug testing (6)
- pre employment
- reasonable suspicion
- random
- post accident
- return to duty
- followup
SAMHSA(NIDA) 5 regulated tests vs expanded
The standard test, includes amphetamines, opium, cocaine, marijuana, and PCP vs the expanded can add in 2-5 other extra drugs depending on employer test
If a drug screen is negative, then ___, if positive, then ___
- send to employer representative but can be informed to the employee
- if positive has to go thru MRO for confirmatory test, if negative after that then drop it but if positive then reach out and if unable to offer reasonable exploration then put in file, but if can offer medical alternative (verified by pharmacy/MD/hospital and reasonable) then negative, MRO must offer employee the option of reconfirmation via a split specimen that must be sent to another HHS lab for reconfirmation
Reasonable alternatives that can cause a positive drug screen (4)
- Hemp containing food products can carry trace THC
- cocaine in opthalmic use or dental work
- OTC cold medicines can have opioids in them, poppy seeds can have opiates in them
- amphetamines can come from pseudoephedrine, adderall, levodopa, etc
In order to be considered for return to duty, how many urine drug screens are needed within a 12 month period?
6 minimum random directly observed
Chronic THC use can take up to ___ days to be cleared from system
90!
Physician Health Program
Program for impaired physicians in the professional sphere that has 92% recovery rate which is far above average for most programs
Opiates vs opioids
Opiates are natural occurring and include only codeine, morphine, and heroin vs opioids are artificially produced and include items such as hydromorphone/oxycodone, metahone, propoxyphene
