7 = comp indep 6 = mod indep (device, safety, time)
5 = supervision/set up 4 = min assist - pt does 75% or more 3 = mod assist = pt does 50% - 74%
2 = max assist = pt does 25 - 49% 1 = total assist = pt less than 25%
0 = activity not performed

7 = ind 6 = device used (special seat, prosthesis) 5 = set up, cues, supervision
4 = touch or steady pt, one leg 3 = both legs, lifting or lowering trunk
2 = lifting and lowering trunk 1 = mechanical lift or 2 people assist

150 ft or greater 7 = ind 6 = mod ind, device used, inc time, safety concern 5 = cues, supervision 4 = contact assist, min assist 3 = mod assist
5 household = ind with 50 - 149 ft, can use device
2 = 50-149 ft with assist
1 = less than 50 ft, or 2 helpers

12-14 with assist
