Two symptoms of typhoid?
GI- diarrhoea or constipation, abdominal pain, rectal bleeding, bowel perforation. Neurological- headache, becteraemia, bradycardia, rose spots

What is the treatment for typhoid?
quinolones (1st line), cephalosporins, azithromycin

What is viral haemorrhagic fever?
infections that cause life threatening disease where blood vessels become leaky. It is extremely rare but must always be considered!
Name two agents that cause viral haemorrhagic fever
lassa, ebola, yellow fever
Two features of viral haemorrhagic fever?
non specific febrile illness, thrombocytopenia resulting in haemorrhagic manifestations, sepsis/shock
20 y/o return from south east asia volunteering following flood. He is fully vaccinated for all endemic diseases.
He has a one-week history of fever and flu-like symptoms. On examination, there is hepatomegaly, red conjunctiva and he is jaundiced. Bloods reveal an acute kidney injury and deranged liver function tests.
What is the most likely diagnosis?
Leptospirosis- common in Asia
Vaccinations for hep B, therefore hep D protection
Vaccination for yellow fever- similar symptoms to leptospirosis minus conjunctival changes
Schistosomiasis transmitted via water borne parasites but not endemic in southeast Asia.
