Questions for patient/parent
Temperature and how was it taken
any contact with sick persons? anyone in household sick?
is patient tolerating liquids?
significant past medical history?
Symptoms
Nausea / vomiting diarrhea irritability cough rash rhinorrhea sore throat abd pain ear pain
PE for fever
Gen: irritable, inconsolable
HEENT - eyes for mucus dc, TMs for erythema and bulging, facial tenderness, nasal dc, mucus membranes moist, if teeth check for cavities gingival swelling abscess, tonsillar exudate erythema.
Heart -
Chest - pneumonia, wheezing, crackles, bronchitis
Abdomen - BSs, masses, HSM
Musculoskeletal - Inspect all joints for swelling and erythema
Skin - ecccymosis, macules, papules, petichia, vesicles.
Neuro - meningeal signs, nuchal rigidity, Kernig’s sign orr Brudzinski sign.
Labs and diagnostics
CBC, UA, Blood Cultures,
CXR, Stool Studies
Diagnosis
Fever >100.4
No identifiable cause. consider high risk. Hospitalize if < 3mo old.
secondary to infection, inflammation, ingestion, malignancy.
Treatment
If not high risk treat with empiric abx therapy. Antipyretic therapy.
