fetal lie position and presentation Flashcards
abdominal palpation is done ..
- Fetal growth and wellbeing
To identify maternal and fetal relationship by determining the lie , attitude , presentation , denominator , position and engagement.
according to nice guidelines when should syphysis fundal height be done
According to nice guidelines symphysis fundal height measurement at each antenatal appointment at 24+0 weeks
lie
- The relationship of the long axis of the fetus to the long axis of the uterus
May be longitudinal , oblique or transverse
attitude
- this is the relationship of the fetal head and limbs to its body
- May be fully flexed , deflexed , partially or fully extended
- When fully flexed the head and spine are flexed , arms crossed over the chest , legs and thighs flexed
In this attitude the fetus forms a compact ovoid , fitting in the uterus comfortable but allowing freedom of movement .
presentation
*This is part of the uterus lying in the lower pole of the uterus
Possible presentations are cephalic , breech , face , brow , shoulder
denominator
Part of the presentation that is used to indicate the position
position
the relationship of the denominator to eight areas of the maternal pelvis
Left , right and direct anterior , left and right transverse , left and right direct posterior
engagement
This occurs when the transverse diameter of the fetal head passes through the brim of the pelvis - this is normally measured in fifths - in direct relation to the ischial spines.
three ways to palpate
look , touch and listen
equipment
tape measure , pinnard measurement
fundal palpation
use two hands to determine the top of the fundus
lateral palpation
back and limbs
pubic palpation
determines presentation and lie of fetus , help to determine engagement
engagement of fetal head is measured in
fifths
