What is epiglottitis?

A

Inflammation of the epiglottis (+ soft tissues surrounding it = supraglottitis)

What are the common causes of epiglottitis now?

A
  • Strep species

* HiB used to be common but since vaccine has reduced

What is the presentation of epiglottitis? Consider common signs and severe signs

A

*Variable, unpredictable and can deteriorate rapidly

Common signs:

  • sore throat
  • Painful swollow
  • fever
  • muffled voice

Severe signs:

  • Resp distress
  • Dysphagia
  • Dysphonia
  • Stridor

Other features:

  • Systemic upset!!
  • Ear pain
  • Cervical lymphadenopathy
  • Anterior neck tendernesss
  • Tripod signs

How do you formally diagnose epiglottitis?

A

*Laryngoscopy is diagnostic test but should be done in theatre/environment equipped for airway management

What is the management of epiglottitis?

A
  • Airway -ENT/Anaesthetics
  • Broad spec IV antibiotics
  • High dose IV steroid
  • IV fluid resus
  • Oxygen
  • Nil by mouth until airway improved
  • PRN adrenaline nebs
