Sudden onset horizontal nystagmus, hearing disturbances, nausea, vomiting and vertigo, often preceded by viral illness:
Acute viral labrynthitis (Vestibular neuritis has very similar presentations but does not cause hearing loss)
Episodes lasting minutes to hours of sudden hearing loss in left ear, associated with dizziness and a feeling of pressure in the affected ear, dx, type of hearing loss and Rinne’s and Weber’s results and mx:
Meniere’s disease, left sensorineural, Rinne AC > BC bilaterally, Weber lateralises to right ear, routine referral to ENT (prochlorperazine acute, betahistine and rehab for prevention)
First-line for otitis externa
Topical corticosteroid + aminoglycoside
Location of branchial cysts:
Smooth swelling in between the sternocleidomastoid muscle and the pharynx
Otitis externa in diabetics
Ciprofloxacin to cover pseudomonas
Treatment for secondary haemorrhage post tonsillectomy 5-10 days PO
Admit to ENT for IV abx, only primary or severe haemorrhages need re-exploration
Ototoxic drugs
Aminoglycosides (genta), aspirin, furosemide, quinine.
