Emergency Procedures Flashcards
Above 80 kt and prior to V1, what should the reason be to “Reject”
Above 80 kt and prior to V1, the take-off should be rejected for any of the following:
• Fire or fire warning
• Engine failure
• Predictive windshear warning
• If the airplane is unsafe or unable to fly
What information do you need after a failure from the checklist? (4)
Flap,
Ref additives,
Watt code,
OLD factor
On a EFAV1 when you hear “____ Eng Fail” what does that tell you?
You can trim X2 (beep beep) because the engine isn’t going to come back
Level back the other way X1 (beep beep)
What is the call after PF has recovered from the stall with airspeed and altitude increasing?
“Clear of warning”
“Go Around Flap”
Flap 2, select FLC
When does EDM come on automatically? (2)
> = FL250 and cabin ALT >14,500
What is the difference in EDM when you manually select it because cabin alt is <14,500 ft? (4)
No 7700
Masks don’t drop for passengers
Emerg descent caution (yellow) not red
Seat belts don’t come on automatically
Cat II land 2 DA 50ft do you need visual at 50 ft to land?
Yes
Which eng fail will give you fuel imbalance issues?
Center tank empty
Right eng fail, because APU and eng feeding from the same side
Motive flow moves extra fuel when trying to keep pressure hi
(Right ac boost pump can’t keep up >82% N1)
At what point is fog considered visible moisture? Distance and altitude?
1 sm and =1500 ft
When do we add the malfunction additive to the Vga and Vac?
Always (accept Bleed failure see AOM1 1.02.21 pg 1)
Change them in first box independent to AT additive
(Write them down before you change them)
What does TAWS stand for?
Terrain awareness warning system
On an engine failure the checklist asked you to consider a fuel leak, when is this a valid concern?
On a flame out or engine failure, cross check your fuel to what you should have.
(Not a engine fire, unlikely to get a fuel imbalance/fuel leak unless tire dam etc)
When doing a diversion (immediate return) what’s the best way to clear the flight plan and get set up quick? (6)
Dir to hard button
Nearest airport (ERRASE Altn)
( you can narrow your search by adjusting the rwy length)
Dep/Arr takes you right to the PERF Arr page to pick your approach
Manual leg
VSPEEDS
DH/mDA
Auto Brake
When doing a flapless landing what will help with the speed control?
Gear down fully configured before GS
(Easy to forget the landing checklist, because sequence is out of order)
What do you have to do for a low energy go around?
Delay the flap retraction until >1,000 fpm
Go around
Flap (>1,000fpm)
Gear up, set speed to Vga
What do you have to do for go around with respect to checklist in a non normal situation?
Must reset all checklists that have non normal carry over things from the failure.
(Normal checklist will reset themselves when they are opened)
What is the gotcha when capturing GS from above with VS?
Make sure in VS and not VVS (turn of VNAV) because some Canadian ILS’s are coded at 3,000 ft for all wpts and won’t descend if given a tight vector and say “descend 2,500 ft”
(Once press TOGA the LNAV and VNAV automatically rearm for the GA)
Common mistake in sequencing after an emergency? RWY PTSD
RWY PTSD
P - Plan - must get atis and decide what you are doing first before T - Talk
What and when are the emergency PA’s for an emergency landing
2 min (FAF) “Emergency Stations” X2
30 sec (500) “brace for landing, brace for landing”
Checklist- Remain seated X2
Evacuate evacuate
What are the three gotchas for WS?
GA WSHR mode will not capture the alt set in MCP must select FLC once clear of warning
Ensure you have T/O WSHR or GA WSHR mode
<400 ft TOGA won’t set full power must set it manually
Cat 2/3 at 20 ft “no retard” comes up what does the FO and CAPT do?
FO “NO RETARD”
CAPT -“ RODGER” Disc and reduce the throttle to idle manually at 20ft
What auto land level is the lowest you land CAT2 with indicated?
Restrictions?
Appr2
but must be manual landing
(80-DH) AP off
What are the predictive WS call outs caution and warning?
T/O
APPR
Caution “Monitor Radar display” (yellow)
Warning - T/O and climb out “WS AHEAD WS AHEAD” (red)
Approach “GO AROUND WINDSHEAR AHEAD” (red)
(Technically must go around for Any warning predictive or reactive, better be sure it’s safe and clear if you decide to continue)
(50-2,400ft T/O
2,300-50 Approach)
JETUPSET nose low and increasing airspeed, what can help you arrest the increasing airspeed?
Spoiler max
Emergency so anything goes
No speed limit on spoiler
How long does it take to go from idle to max power at 35,000 ft?
35 sec
(Because Hi bypass ratio eng)
Why must we brief the CAT2 from the QRHN?
Legal reasons for recording
When is Predictive and Reactive Windshear active/inhibited?
Predictive
0-80 Kts ON
80 Kts - 50 ft OFF
50 ft - 2,400 ft ON
Reactive
10ft - 1,500 ft RA System ON
Considerations for high winds or reported TS for T/O? (6)
WAT - (windsheer/inversion ON) =TO
Minimum flap
Select longest runway
Brief WS recovery with T/O briefing
Request 30 sec on runway for Radar check
Set radar to WX + TURB on CTP
Biggest Mistake on WS spot session in sept?
WS turn AP on if possible makes it much easier (T/O at 400)
TRIM as you hand fly
“Clear of warning” = FLC asap
Flap1, Flap 0
(Controls the speed better to avoid the over speed)
Mistake on last eng fail in the go around?
Set Vfe at 1,000 AAE instead of waiting for Acceleration Altitude
( wait for FLC)
What is the only reason you should relight an engine?
Only volcanic ash
Because fadec already attempts a relight on its own
Once you are on LOC on an approach (2,000ft RA) how can you get out laterally, ie predictive WS, PMA
TOGA
Then ALT, set an alt, FLC
What to you have to do if you have entered the Vref manually (icing, turns blue)
Must delete the Vref and then it will recalculate
Good idea when you get a eng failure?
PF puts fuel page up on PFD2
(Especially if Right engine failure)
What is the call to RED 1 after touchdown when stopped
Brakes Set
L Eng secure
right Eng running
ready for inspection
Tips for CAT 2/3 briefings
QRHN pg 27 (required equip)
pg 29 (vis req)
pg 31 (Cat2/3 brief)
Read in order
(Low vis lead in lights go from alternating white and green to green when clear of runway)
(Engage FO by saying then you say…)
Plan to do a visual approach on your command sims. What can help?
Dir to airport
DEP/ARR - select runway not app
(Gives you 5 nm off approach)
Speeds
DH
Auto Brk
10nm at 3 deg is 3,000 ft
5 nm 1,500 ft
What is the Threat and error management system TEM?
Threat and Error Management (TEM) is used to handle Expected (proactive)
and Unexpected (reactive) threats by proactively identifying, managing,
and correcting any threats and errors encountered before there is a significant impact on flight safety.
Define AOV
Area of vulnerability
Phase of flight where aircraft is most likely to enter a UAS (Undesired Aircraft State)
Pilot incapacitation
Aviate
- Assume Control with AP On
- Select side stick priority on glareshield
Navigate
-Maintain Flight Path
Communicate
-“In Charge Flight Attendant Report to the Flight Deck”- Declare Emergency with ATC
Three R’s
-Restrain
-Remove
-Replace
PAN PAN
Fuel spill actions
Stop fueling
Advise STOC
Advise ATC
Advise Cabin crew
Rapid deplanment if spill 1.5 M in any direction or > 12mm in depth
“Deplane immediately and leave your personal belongings”
What is the Acronym for who to contact after a incident or accident?
DDDAD
Duty Pilot
Dispatch
Deactivation - CVR
ASR
Debrief
What should you do if you think you are getting an eng surge?
Reduce the power on that eng
If cruise increases good Eng MCT
Drill
Consideration for unruly passenger checklist in the QRH?
This is company rating 1-4, does not align with USA rating, do not give it to ATC
(Level 3 in USA is Hijack requesting F18)
What is PTI?
Positive Target Identification - Bomb threat assessment
Is used to differentiate between a threat and a hoax by assessment committee (Dispatch)
What is a gotcha when equip is inop (emerg mode only one GPS) which restricts what kind of minimums we can use, like LPV - LNAV only min
Don’t forget to down grade the type of approach you can do in DEP/ARR
Where does the RED 1 freq go after landing on runway for inspection?
Must go in VHF 2
Advise FO
How do you quickly determine ALT for descent when 45nm out?
Distance X 300
Eg 45 nm X 3 = 13,500 ft
Eng failure in cruise Drill (new)
Increase on good eng
MCT check
Disconnect AT
Speed to Green dot
Turn if required (30 deg and offset 5nm if Oceanic)
Altitude for drift down
FLC when at GD (then and only then arm AT)
Speed adjust to GD
Checklist
In a “time situation” what is a good technique for briefing IC
“IC FA report to the Flight Deck”
Do you have your card?
Are you OK?
TTTBAD
Please read it back
You lose AP what are the considerations? (3)
No RVSM 290-410
No RNP AR
No Cat3 (no autoland)
What could you do if you lost all airspeed/alt information?
FMS/POS/GNSS to get AS and
FMS/POS/GNSS INFO to get ALT information
Display 2 FMS pages on second display of pilot flying (REV if req)
What do you instruct the FO to do after the evacuation checklist list is done?
QRH/phone/Full Uniform
Gather passengers 300 ft on the upwind side
I will make sure everyone is off the airplane then I will join you
What is done after EMS takes over the passengers?
DDDAD
Gather crew in quiet place (chapel/manager office)
Speak to no media (statement in back of QRH)
Non crew hotel
Get food
debrief (starting with most junior member)
What is the gotcha for Kelowna OEI operations
Climb gradient 370/nm 3500 ft means you cannot level off at 1,000 ft like the SOP for Eng failure
TO thrust for take off
Don’t use FMS speed in GoAround, not coded
What is done immediately after an emergency/evacuation?
Evac Proc (cell,QRH, logbook)
Safe guard life and property
Notify authorities (ATC) dispatch
Notify nearest Aircanada Station
Wait for station manager relief (A/C not moved/spectators away/no press)
DDDAD
Gather Crew together (hotel/office) Medical care for crew
Crew writes separate statement
